What the Statute Requires

The four documents Section 553.791 requires, with the subsection for each.

It must be updated within 1 business day after any change. After construction has started, a provider can be added only for single-trade inspections on one- and two-family dwellings, with notice before the next scheduled inspection. s. 553.791(5)

Statewide forms are adopted by the Florida Building Commission. Local building departments may not require additional forms beyond those required at provider registration (s. 553.791(2)(e)).

How to Complete Documentation

Practical points for keeping private provider documentation accurate and complete.

1

1. Use the Commission Forms

The NTBO, inspection records, and certificate of compliance use the forms adopted by the Florida Building Commission. A building department may not alter the notice form or require additional forms beyond those required at provider registration.

2

2. Match the Permit

Keep the scope of work on each document consistent with the permit application. This is part of the normal permitting process and avoids review delays.

3

3. Complete Every Field

Complete each document as the form requires. Who signs, and whether a statement is made under oath, depends on the document: the owner or contractor signs the NTBO, and the provider signs the affidavit and certificate.
